Each participant MUST submit the following prior to beginning practice:

2010 Participant Contract and Parental Consent Form
2010 Physical Fitness and Medical History
2009-2010 Official School Report Card COPY or 2010 Home School Eligibility Form
Official Birth Certificate COPY
Registration Fee ($80)
