Eastern Pender County Pop Warner Association will offer
several opportunities to register for the 2011 season.
Each participant MUST submit the following prior to beginning practice:
2011 Participant Contract and Parental Consent Form
2011 Physical Fitness and Medical History
2010-2011 Official School Report Card COPY or 2011 Home School Eligibility Form
Official Birth Certificate COPY
Registration Fee
